To apply to become a UVMC volunteer, please download and complete the volunteer application form and mail it to: Felicia Eriksen, Volunteer Services Manager, Ukiah Valley Medical Center, 275 Hospital Drive, Ukiah, CA 95482. Once your application has been reviewed, a member of our volunteer staff will contact you to discuss your interest.
