Application process

Step One: Complete online application. Step Two: Watch the video. Step Three: Read the UnityPoint Health-Finley Hospital Volunteer Handbook. Step Four: Read and Review Volunteer Orientation PowerPoint. Step Five: Complete the handbook and PowerPoint acknowledgement form. Step Six: Read and complete: HIPAA information and Quiz; Personal Appearance Policy; Information Security Agreement; Certificate of the Right to Know; Blood Borne Pathogens Education; Finley Hospital Fact Sheet; Background check for those 18 and older. Step Seven: Request an in-person interview. Step Eight: Complete your health screening. Call to set up an appointment for your health screen at (563) 585-1290. Review the Volunteer Health Screen Information and Requirements. Complete the following forms which MUST be brought to your health screen appointment along with your most up to date IMMUNIZATION RECORDS: Volunteer Staff Health Screen; If you are under the age of 18 years please also have your parent complete: Volunteer Screening Minor Permission Form. Step Nine: Set-up in-person orientation and department training.
