Application process
Call for an appointment (Indianapolis 317-338-2268 and Carmel 317-582-7158); Obtain two tuberculosis tests (we provide at no charge); Sign a confidentiality statement; Obtain parental consent if you are under 18; Have a blood screening (we provide at no charge); Consent to a criminal background check; Attend a training session
