Application process

Application; Consent for Criminal Background Check/ Sex Offender Registry Check; A Medical Reference Form. These 3 pieces must be completed and returned to Judi Trenholm, Pen Bay Volunteer Director; Employee Health (EH) pre-placement screening form. Complete and bring this form to your EH appointment. Also in your packet, you will find a copy of the PBH Confidentiality Agreement and the Maine Health Values in Actiondocument. We will review these together in more detail but ask that you take a moment to review these expectations before applying. Please review, sign and return both forms with your application. If applicant is under 18 years of age, parent or legal guardian must also sign all documents.
