Adult Volunteer Application Process - Step 1: Application Process - Complete a volunteer application online; Submit (2) satisfactory references from non-family members; Complete the following (3) background check forms and bring to your interview: Delaware Adult Abuse Registry, Delaware Child Protection Registry, Volunteer Pre-Check Release. Step 2: Appointment for Interview - Once we have received your application and references, you will be contacted for an interview. Remember to bring the (3) completed background forms for processing. Step 3: Orientation & Background. Step 4: Complete Forms - Confidentiality of Information Statement, Photo Consent Form, Volunteer Sign-Off Sheet, Step 5: Notify Volunteer Services - Once you’ve completed the previous steps and PPD test, please contact Volunteer Services either by email or phone at 302-629-6611, ext. 4955. Volunteer Services will coordinate your first day of volunteering and get you started as quickly as possible; Summer Teen Application Process - Step 1: Submit Application. Step 2: Required Paperwork - Parent/Guardian Paperwork: Parental Approval for Teen Application, Parent/Guardian Consent for TST Treatment, Emergency Data Sheet, Summer Teen Availability Form, Parent/Guardian Reading Materials: Tuberculosis Surveillance Guidelines, Teen Applicant Paperwork: Reading Materials: Basic Self-Learning Packet, Summer Teen Code of Conduct & Dress Code, Volunteer Guidelines & Expectations, Standards of Performance, Code of Ethics and Business Conduct Handbook, Dress Code Guidelines,Tobacco Free Policy. Please bring the following completed paperwork to your interview: Basic Self-Learning Test, Three (3) Signed Letters of Recommendation (Must be from teachers, coaches, clergy or community members - no family members), Confidentiality of Information Statement, Photo Consent Form, Summer Teen Sign Off Sheet, $15 EXACT CASH for NHS Volunteer Polo Shirt.
