Application process

1. Please complete forms: Application; Parent/guardian consent form (teens only); Include a copy of immunization record, if available. Flu vaccine is mandatory from December to April; Three reference forms. Please ask three adults to complete a form for you. The reference forms may be mailed back with your application forms Mail the completed set of forms to: Johnson Memorial Hospital, Volunteer Services, 201Chestnut Hill Road, Stafford Springs, CT 06076 Or fax to 860-684-8189. 2. Interview. When we have received your application, we will call you to schedule a time for you to meet with us. We will discuss your department interests, schedules and availability. Allow approximately 30 minutes for this appointment. Adults - you will be asked to sign an authorization for JMH to conduct a background check. At your meeting with Volunteer Services, we will review with you information on how to schedule an appointment with the Johnson Occupational Medicine Center (JOMC) located in Enfield for health screening. 3. Health Screening. You will be required to make appointments at JMH for two TB tests (also know as PPD’s). When completed we will receive clearance that you may start volunteering. There is not a charge for the PPD’s. 4. Orientation.
