1. Obtain an application from the Volunteer Office, the main Information Desk or online: www.FHZeph.org. 2. Upon completing the application, call (813) 779‐6256 or come by the Volunteer Office to set up an appointment for an interview. 3. After satisfactorily completing the interview, a background check request will be submitted through Human Resources. 4. After clearance of the background check, the Applicant will be scheduled for drug, nicotine and tuberculosis (TB) screening. These screenings will be scheduled with Employee Health at no cost to the Applicant. Adults will need two skin tests, teens (under the age of 18) only one. Teens must be accompanied by a parent or guardian. If a TB evaluation has been completed within a year's time, a copy may be presented to the Employee Health nurse. Flu shots are required once a year. 5. Upon clearance from Employee Health, the Applicant will be scheduled to attend the allday orientation. 6. After completion of orientation, the applicant will need to schedule an appointment with the Volunteer office where they will receive an assignment, badge and shirt.
