Application process

You can download the volunteer application and email or standard mail it to: 1454 Madison Avenue Immokalee FL 34142 attn: Nancy Freeman. The Healthcare Network will need your complete vaccination record at the time that you turn in your application. You will also be attending a half day of orientation. Our organization is required to have your immunization information on file along with the record of attendance at orientation prior to having volunteers at any of our locations. Vaccines needed: Measles; Mumps; Rubella; Varicella; Pertussis; PPD (skin test for Tuberculosis, required annually for health care workers); Hepatitis B (for those doing direct patient care). Volunteers holding a professional health care license and wishing to work with patients will need to provide a valid Florida professional license, and other credentials.
